Shop as normal, placing items in your cart When you are finished selecting items, go though the check out as normal adding your address details etc. Before final order confirmation and payment, you will have the option to 'review your order for mail/fax orders'. This will give you a printed form with order and delivery details already done For credit card payment, fill out the required details including card number, expiry date, name on card and sign form Mail or Fax the order to address/number on the form
